Verification Information
If your application is selected for verification, an e-mail notification will be sent to your registered KU email address. Please print, complete, and submit the appropriate Verification Worksheet to the OSFA along with signed copies of all applicable Federal tax returns, W-2s, and 1099 forms. If you are a dependent student, you will need to provide your parents' or guardian's signed Federal tax returns, W-2s, and 1099 forms. Please do not submit tax documents unless requested by the OSFA.
