2009-10 Financial Aid Forms

Fall 2009, Spring 2010, and Summer 2010

• Cancellation of Aid Request Form Request your aid be cancelled at KU for a specific term or for the 2009-10 academic year.
• Childcare Grant Application and Budget Adjustment A limited number of Childcare Grants supported through the Educational Opportunity Fund (EOF) are available for eligible KU students for the 2009-10 academic year. The priority deadline to submit the application is September 25th. Recipients of the grant will be notified via email by mid-October.
• Computer Budget Adjustment Request Form Request an adjustment to your Cost of Attendance for the purchase of a computer or software.
• Documentation of Independent Status Form A form utilized by students who are asked to document their independent student status.
• Graduate PLUS Loan Request Form Request form for graduate/professional/law students seeking a loan in addition to their Stafford loan(s).
• International Student Needs Analysis Form A worksheet for international students who want to be considered for need-based scholarships, the KU International Student Tuition Grant, and/or the EOF Childcare Grant.
• Legal Dependent Form A form utilized by students who are asked to document support for a legal dependent.
• Non-Degree Financial Aid Request Form Request financial aid if you are a non-degree seeking student and meet certain eligibility criteria.
• PLUS Loan Request Form Form that parents who want to borrow a Parent Loan For Undergraduate Students (PLUS) must complete.
• Reinstatement of Financial Aid Request Form Request your aid be reinstated for the year or a specific term. A request for reinstatment does not guarantee that your original awards will still be available.
• Request for Additional Loans Form – Graduate/Professional/Law Students Request additional Stafford loans for the 2009-10 academic year if your loan eligibility has increased or if you did not previously accept the full amount offered.
• Request for Additional Loans Form – Undergraduate Students Request additional Stafford loans for the 2009-10 academic year if your loan eligibility has increased or if you did not previously accept the full amount offered.
• Special Circumstances Re-Evaluation Request form Request your income information or Cost of Attendance (COA) be re-evaluated based on extenuating circumstances.
